To receive your rebate check by mail:

• Hand-print your name, address, city, state, zip code, e-mail address (optional) and the number of participating products purchased on the official offer form;
  
• To receive a $3.00 rebate check , send original register receipt dated between September 1, 2008 – October 31, 2008 showing proof of purchase of Mobil Delvac 1300 Super 15W-40 one-gallon jug(s). Individual can receive a $3.00 rebate check for each Mobil Delvac 1300 Super 15W-40 one-gallon jug purchase made between September 1, 2008 – October 31, 2008, up to a maximum of twelve (12); or
  
• To receive a $15.00 rebate check , send original register receipt dated between September 1, 2008 – October 31, 2008 showing proof of purchase of Mobil Delvac 1300 Super 15W-40 five-gallon oil pail(s). Individual can receive a $15.00 rebate check for each Mobil Delvac 1300 Super 15W-40 five-gallon oil pail purchase made between September 1, 2008 – October 31, 2008, up to a maximum of three (3), and mail to:

All rebate requests must be received by December 15, 2008. Limit of up to 12 Mobil Delvac 1300 Super 15W-40 one-gallon jug $3.00 rebates per person and per household; and of up to three five-gallon Mobil Delvac 1300 Super 15W-40 oil pail $15.00 rebates per person and per household. Rebates can be combined and mailed together. Allow approximately 6-8 weeks for delivery following receipt of all required documentation. Void in Rhode Island and where prohibited, restricted or taxed. Offer valid only to individual consumers; requests from groups, clubs or organizations will not be honored.
